This resource aims to provide a high-level summary to assist individuals and families in learning about and connecting to transition services for which they may qualify to receive as they prepare for and enter life beyond high school. It also serves as a tool for disability support resources and agencies to foster connections with transition services, assisting individuals with disabilities achieve and maintain employment, further pursue further education or training, and other postsecondary goals.
